new form. Once there, please fill out the
following fields:
- School name.
- Your name, cellular telephone number,
other phone number and e-mail address.
- Preferred method of contact.
- List each HOME Scrimmage only.
- List alternate contact information
(assistant coach, J.V. Coach if J.V.
Scrimmages included).
You may submit the form three ways:
- Save the form as an Excel document by
clicking "save as." Change the name of the
document to your school name (dot) xls -

- E-mail the completed form to

- Print & Fax the completed form to (516)
608-1367 or
- Print and submit at coach's meeting
Board 41 will assign officials to act as
liaisons with the coaching staff. We will make
every effort to supply official's to your
scrimmages, however, we cannot guarantee
coverage. Once a liaison has been assigned to
your specific school, he will contact you and
confirm assignments. We ask that you supply
working officials with locker rooms, showers,
and water. In the event of a cancellation, or
change, please contact your liaison immediately.
In addition, please e-mail your liaison with
complete list of all officials that worked your
scrimmage. Board 41 will be tracking attendance
in an effort to improve our coverage. Thank you
for your anticipated cooperation.
